Soul shards

Right well… my PCs never die much and when it happen they have been fortunate to have friends around to bring them back.
I remember that (this may have changed tho) from 1-3 lvl a PC had a soul shard or something, but what happens after that. Is there a way to come back after that lvl without the help of friends or DM? And what are the penalties?

You can come back with the help of the angel in the Fugue (who I will forever think of as Tyrael, even though he's not) for double the normal XP loss from dying.
