Grorg probably just comes across as "just another simple minded half-orc" to most, except that he's bigger sized than most. That may have something to do with Grorg being seen with food of some kind or other in his hands everytime one sees him (or that it's really divine providence that one catches him in the act of eating whenever one sees him).
To critters, Grorg is something to be feared. If one could actually understand critter-speak, they would hear him being referred to as "an axe-wielding butchering bastard maniac"…which may or not be far from the truth. Grorg seems to always sharpening an axe, and talking about "a chop shop", "preparing food", and the like. The nightmare of chickens and other edible animals, indeed - or anyone with a sense of decorum.
Having said that, though, when push comes to shove, and you're surrounded in a bar fight, there's no one you'd rather have backing you up. But before you do, it makes plenty of sense to talk to him first and find out more...
